Wrinkle relaxing injections are a purified protein produced by a specific strain of bacteria. It was developed during the 1970s – 1980s to treat blepharospasm (spasm of the eyelids) and strabismus (squint).

  • The use of anti-wrinkle injections in such situations (i.e. for patients on immunosuppressive medication for chronic disease), shall ‘not’ be an issue.

If tiny forehead wrinkles reappear 3 months after anti-wrinkle injections, does this mean the dosage was not enough?

  • What you’ve described is within normal tolerances. 
  • The effect from anti-wrinkle injections (i.e. muscle relaxation) typically lasts 90-100 days, but with a range of 2-6 months (for different people and depending on various factors).
  • Importantly, you should note that the effect of anti-wrinkle injections, irrespective of brand, starts to ‘gradually’ decrease after it produces its maximum outcome (which is typically 2-4 weeks after treatment).  Accordingly, I typically tell patients that they’re likely to see some fine wrinkles start to appear after 2 months; 
  • Please note that whilst unduly high doses won’t increase the ‘maximum’ possible duration of action in a particular individual, being ‘underdosed’ (inadequately treated) means that the wrinkles shall return quicker (because the muscle has not been paralyzed enough).


Can you receive under eye/tear trough filler and under-eye anti-wrinkle injections the same day?

  • There is ‘no’ issue with treating the muscle around the eye (orbicularis oculi muscle near to the lower eyelid) on the same day as giving fillers to the tear troughs. 
  • However, we need considered judgement when treating tear troughs (due to the hygroscopicity of the filler, i.e. the innate ability of temporary fillers to attract water) particularly in the context of reducing the tone on orbicularis oculi – the risk being transient ‘fullness’ of the under-eye area, which would almost certainly look worse than not treating the area. 
  • Therefore, make sure you go to a cosmetic physician or cosmetic surgeon who truly has adequate experience with such treatments.

Can I get Velashape on my torso after under-eye fillers? Do they impact each other such as the rate of filler breakdown?

  • Yes, you may get the treatment with Velashape without the concerns you expressed. 
  • ‘Had’ the desired treatment-area been the same, then it would have been better to use the energy-based device prior to getting the fillers; however, in your case, given the completely different area to be treated, there shall be ‘no’ deleterious impact or interference.

Is it safe to get anti-wrinkle injections if you already have chin filler?

  • There are ‘no’ issues to the chin being treated with both anti-wrinkle injections and fillers. 
  • It is important to consider the relative (and absolute) contours and proportions of the chin/ jawline, with respect to the rest of the face.

PRP and effectiveness for hollowness and skin redness?

  • There is a lot of marketing that surrounds PRP, in part due to a lack of standardization of research into this modality. 
  • The reality is that it does ‘not’ do very much when injected for aesthetic reasons to the face.  After a few minutes, it gets washed out by the blood flow.  
  • It is most certainly ‘not’ a meaningful solution for hollowness! 
  • PRP may be used in conjunction with other modalities and products. When used in isolation, however, all PRP does is to give ‘very temporary volumisation’ with no other tangible change.

Acne is a common skin disease involving blockage of the pilosebaceous units (oil glands and hair follicles in the skin), that frequently leads to inflammation.  The key factor behind acne is genetics, and around 1 in 11 of the population suffer from acne. The typically affected areas are the face and back. 

During teenage years, males suffer more with acne than females; however, during adulthood, women suffer more (and some women suffer from increased breakouts during pregnancy).

What Are The Causes Of Acne?

Addressing these factors shall reduce the likelihood of breakouts: 

  • Bacterial load (of Cutibacterium acnes) found in the skin; 
  • Reducing the amount of oil (sebum) that is produced; 
  • Reducing the amount of dead and/ or thickened skin on the surface (hyper keratinisation blocks the hair follicles).

What Are The Options Available For Dealing With Scarring Due To Acne?

  • There has been a lot of misinformation about the use of Accutane/Roaccutane (prescribed for severe acne, especially in younger people) and the fragility of the skin.  
  • Isotretinoin works at the level of the oil glands/hair follicles of the skin and it suppresses inflammation of the skin.  It is one of the few, clearly-proven approaches to addressing acne, and it results in significant improvement (typically >50%) after just 2-3 months’ use.  However, dry and fragile skin (with an increased likelihood of cracking) is a possibility. The skin is also more photosensitive, i.e. vulnerable to the damaging effects of UV light as found in the sun, so the use of something like Amaranté PNP (www.amarante.com.au) for its moisturizing and SPF benefits, is recommended. 
  • A common question that is asked is whether lip fillers may be administered whilst somebody is on isotretinoin.  Of course, the skin’s quality and the pros/ cons need to be considered by the doctor, but the answer, as a rule-of-thumb, is: ‘not’ an issue.  
  • If seeking fillers in other areas, then the question is whether there is ‘active’ acne or not.  If there is active acne, there shall be a higher risk of contamination if injecting through the inflamed area. 
  • Regarding receiving anti-wrinkle injections (botulinum neurotoxin type-A) when somebody is on treatment for acne, there is ‘no’ issue whatsoever.
